He placed it on my table with the satisfaction of a man who had outsmarted biology itself. “Doctor,” he said, “now I don’t have to eat fish anymore.” I looked at the bottle, then at him, and wondered how a sardine would react if it knew its lifelong ambition had become a shiny capsule. Somewhere in the Arabian Sea, millions of fish were swimming peacefully, blissfully unaware that humans had promoted them from dinner to dietary supplements. Evolution must occasionally laugh when nobody is looking.

The question is understandable. Fish smells. Fish has bones. Fish requires cleaning, cooking, and occasionally negotiating with an enthusiastic fishmonger who insists today’s catch was swimming five minutes ago. Capsules, on the other hand, are obedient. They neither smell nor stare back. Swallow one with water, and health appears wonderfully convenient. It is hardly surprising that one of the most searched nutrition questions today is whether fish oil can simply replace fish. The answer is fascinating because nature rarely rewards convenience with completeness.

Before deciding between fish and fish oil, we need to solve an even bigger mystery. Fish are celebrated for omega-3 fatty acids. Fair enough. But who taught the fish to make omega-3? Here comes the first surprise. They never learnt. Fish are innocent middlemen. The real manufacturers are microscopic marine algae floating quietly near the ocean’s surface. Tiny organisms produce EPA and DHA. Small fish eat them. Bigger fish eat smaller fish. Humans eventually eat the bigger fish and congratulate the fish for a job actually performed by algae. Credit, it seems, has been travelling up the food chain for millions of years.

That tiny detail changes everything. Fish are not magical factories. They are remarkably efficient storage vessels. Nature chose them because fat protects these delicate omega-3 molecules until another creature needs them. It is an elegant biological relay race. Nobody advertises. Nobody prints labels. Nobody hires influencers. The ocean simply continues a nutritional partnership that began long before human beings invented words like antioxidants, superfoods or wellness retreats. Every time we discover one of nature’s clever ideas, we immediately assume we invented it.

The stars of this story are two omega-3 fatty acids called EPA and DHA. Their names sound like software updates, but they help build some of the most sophisticated structures in the human body. DHA forms an important part of the brain and retina. EPA helps regulate inflammation and supports cardiovascular health. Scientists continue discovering new roles for these remarkable fats, from maintaining nerve function to producing specialised molecules that help the body resolve inflammation rather than merely suppress it. The body does not treat omega-3 as an optional luxury. It files it under essential infrastructure.

Curiously, fish never became famous because of omega-3 alone. Long before laboratories measured fatty acids, fishing communities across the world had already observed something important. People who regularly ate fish often remained active well into old age. Their observations travelled through kitchens, not conferences. Modern research later strengthened many of those impressions. Populations that regularly consume fish tend to show lower rates of cardiovascular disease, while adequate DHA intake during pregnancy supports fetal brain and eye development. Science deserves credit for measuring the phenomenon. Experience deserves equal credit for noticing it first.

This brings us to the central question. Is swallowing fish oil the same as eating fish? Imagine attending a full symphony orchestra and then listening only to the violin at home. The violin is beautiful. It may even be the star performer. Yet something unmistakable has disappeared. Fish is exactly like that orchestra. Along with EPA and DHA, it provides complete protein, vitamin D, vitamin B12, selenium, iodine and several other nutrients that quietly interact with one another. Nutrition rarely works through solitary heroes. It behaves more like an experienced cricket team where victory depends on partnerships rather than one spectacular batsman.

Then comes another surprise, particularly for Indians. Whenever omega-3 is discussed, somebody inevitably mentions salmon as though every healthy fish must carry a foreign passport. Salmon is undoubtedly nutritious. Wild Atlantic salmon and Pacific salmon naturally contain generous amounts of omega-3. There is only one small geographical inconvenience. They do not swim through the Arabian Sea or the Indian Ocean. The salmon prominently displayed in many Indian stores is usually imported, farmed or frozen. There is nothing inherently wrong with that. But believing that health arrives only after crossing international waters is nutritional snobbery dressed as science.

Walk instead into a traditional fish market in Mangaluru, Kochi, Karwar or Mumbai. Standing quietly in crushed ice is Bangda, the humble Indian mackerel. Nearby lies Mathi, the Indian oil sardine. Neither has an expensive marketing department. Neither appears frequently on glossy magazine covers. Yet both are nutritional powerhouses loaded with omega-3 fatty acids. In fact, Indian oil sardines often rival or exceed several fashionable imported varieties. Nature has an amusing habit of hiding extraordinary value inside ordinary-looking packages. It has been doing so long before luxury supermarkets arrived to explain food to us.

If I had to choose only a handful of fish for nutritional value, the list would surprise many readers. Bangda would comfortably occupy the front row. Mathi would stand proudly beside it. Anchovies, though tiny enough to escape attention, are another excellent source. Hilsa remains legendary where available. Rawas, our so-called Indian salmon, deserves appreciation despite not being closely related to Atlantic salmon. Surmai offers good nutrition, though its omega-3 content varies more than people assume. Rohu and Catla contribute too, especially for those who prefer freshwater fish, although oily marine fish generally remain superior sources. The healthiest fish in the market is not always the one with the highest price. Markets, like social media, occasionally mistake popularity for excellence.

The greatest irony is this. Many patients happily spend thousands of rupees every year on imported supplements while bargaining fiercely with the fish seller over twenty rupees. They trust a capsule that travelled halfway around the world yet suspect the Bangda caught off the Konkan coast. After three decades in clinical practice, I have learnt that modern nutrition suffers less from lack of information than from misplaced admiration. We often assume health must arrive in sophisticated packaging. Meanwhile, the fish continues its quiet work, asking for nothing more than a place on the plate.

The story becomes even more interesting in the kitchen. Patients often ask whether frying destroys all the precious omega-3. If that were true, entire coastal civilisations would have been nutritional disasters centuries before chemistry existed. Ordinary cooking causes only modest losses. Fish curry, steaming, baking and shallow pan-cooking preserve most of the valuable fats. Repeated deep-frying at very high temperatures is another matter. Oil ages just as people do. After enough reheating, it becomes less graceful and considerably less useful. Fortunately, your grandmother’s fish curry is usually a better nutritionist than the fryer in a fast-food outlet.

Another anxiety deserves retirement. Mercury. Every discussion about fish eventually arrives there, usually with enough panic to empty the sea. Mercury is a genuine concern, but context matters. Large predatory fish that live longer accumulate more mercury because they spend years eating smaller fish. This process, called biomagnification, is one of ecology’s quieter lessons. Smaller oily fish such as sardines and anchovies generally contain far less mercury while delivering abundant omega-3. Nature occasionally rewards modesty. The fish that attracts the least attention often carries the greatest nutritional reputation.

Then there is farmed versus wild fish. The debate has generated more heat than clarity. Wild fish usually eat their natural diet, while farmed fish consume specially prepared feed. The nutritional differences therefore depend greatly on farming practices. Here comes another delightful surprise. Many people assume salmon is naturally bright pink. Wild salmon owe their colour to carotenoids obtained from tiny crustaceans. Farmed salmon often receive the same pigments through their feed because consumers expect salmon to look pink. The fish has not become dishonest. Human expectations have simply become very particular. Somewhere in marketing, colour quietly became a nutrient.

Now let us return to the capsule sitting patiently on the pharmacy shelf. Fish oil is not a fraud. It has an honourable place in medicine. The difficulty begins when it is asked to perform duties it never accepted. Fish oil supplements are useful for people who genuinely cannot eat fish, for some pregnant women who need DHA but avoid seafood, for strict vegetarians choosing algal alternatives, and for individuals with very high triglyceride levels who are prescribed concentrated omega-3 preparations. That last point matters because prescription omega-3 is medicine, not merely nutrition. There is a difference between carrying an umbrella because it may drizzle and because the weather department predicts a cyclone.

Quality, however, varies enormously among supplements. Two bottles standing beside each other may contain very different amounts of EPA and DHA despite looking almost identical. Some use triglyceride forms that are absorbed efficiently. Others contain ethyl esters that behave differently in the body. Oxidation during storage can reduce quality, which is why that unmistakable fishy smell after opening a bottle is not always a compliment. Capsules deserve the same scrutiny we give vegetables. Freshness is not an old-fashioned obsession; it is chemistry wearing common sense.

People often ask me how much omega-3 they actually need. For most healthy adults, eating one or two servings of oily fish each week comfortably provides what the body requires. Those who do not eat fish may consider around 250 to 500 milligrams of combined EPA and DHA daily after discussing it with their physician. Pregnant women are usually advised to ensure adequate DHA, commonly around 200 to 300 milligrams daily, because the developing brain is an astonishingly demanding architect. Children generally benefit more from learning to enjoy fish than from learning to swallow capsules. The happiest nutritional habit often begins long before the first school examination.

Vegetarians naturally raise an important objection. If fish obtain omega-3 from algae, why not go directly to the source? It is an excellent question. Flaxseeds, walnuts and chia seeds provide alpha-linolenic acid, another member of the omega-3 family. The body can convert part of it into EPA and DHA, but the conversion is surprisingly modest. Algal DHA supplements bypass this limitation and provide a genuinely vegetarian source of long-chain omega-3. The fish, in this story, is simply a very successful distributor. The original manufacturer continues floating unnoticed in the ocean.

One Ayurvedic observation about fish deserves far more attention than it receives. Classical texts did not classify fish merely by size or taste; they classified them by where they lived. Fish from fast-flowing rivers were described differently from those inhabiting marshes, ponds and stagnant waters because habitat was believed to influence digestibility and bodily effects. They also observed that small fish (Kṣudra Matsya) are generally lighter to digest than large fish, an idea many coastal families still follow without knowing its classical origin. Most remarkably, Ayurveda insisted that fish should be eaten as fresh as possible, recognising that a fish changes rapidly once removed from water. Centuries before microbiology explained bacterial growth and food spoilage, Ayurvedic physicians had already reached the same practical conclusion through careful clinical observation: the life of the fish does not end at the catch, but its quality begins to decline from that very moment.

Fish or fish oil, then?  If you enjoy fish and have no medical reason to avoid it, the fish usually wins. It brings protein, vitamins, minerals and omega-3 together in a partnership that no capsule can completely imitate. Fish oil remains a useful substitute when genuine need exists, but substitutes become troublesome when they begin claiming the throne. Perhaps that explains why the Arabian Sea has never opened a supplement factory. It has been producing the original formulation for millions of years, one Bangda and one Mathi at a time, without ever needing a glossy label.

A Few Questions Patients Ask Me Almost Every Week

One question arrives with remarkable punctuality. “Doctor, if I eat fish today, do I still need my fish oil capsule?” For most healthy people who eat oily fish once or twice a week, the answer is usually no. Food should remain the first choice. Supplements are exactly what the name suggests—they supplement. 

Another popular question concerns burping. Fish oil capsules occasionally announce their presence several hours after breakfast with what patients politely describe as “fishy repeats.” This usually happens because the capsule dissolves in the stomach and the oil travels upwards with belching. Taking it with meals or choosing better-quality preparations often helps. Fish, interestingly, rarely argues with you after lunch unless the cook had an unusually adventurous day.

People also ask whether one fish is enough for life if it is particularly large. Nutrition is wonderfully democratic. One magnificent kingfish eaten once a month does not compensate for three weeks of nutritional absenteeism. The body appreciates regular deposits more than spectacular one-time investments. Health behaves less like winning a lottery and more like paying a recurring monthly instalment.

Storage matters more than many realise. Fresh fish should smell of the sea, not of suspicion. A strong ammonia-like odour is not maturity; it is decomposition. Frozen fish is perfectly acceptable when handled properly, but repeated thawing and refreezing quietly damages texture and quality. Fish oil capsules also dislike heat, light and prolonged exposure to air. Even nutrients have retirement plans if treated badly.

A delightful historical curiosity deserves mention. Archaeologists have found evidence that coastal communities were eating fish thousands of years before agriculture became widespread. Long before supermarkets organised seafood into neat refrigerated shelves, rivers and oceans had already become humanity’s earliest food markets. Civilisations rose beside water for many reasons—transport, trade and fertile land among them. Fish simply made the neighbourhood even more attractive.

There is another irony worth smiling about. The smallest fish often perform the biggest nutritional service. Sardines and anchovies rarely receive glamorous documentaries or luxury restaurant menus. Yet they quietly deliver generous amounts of omega-3, high-quality protein and minerals while generally remaining lower in mercury than many larger species. In nutrition, as in life, loud publicity and lasting value are only distant relatives.

That explains why every few years the world discovers a new miracle food, prints colourful labels, invites celebrities, creates premium packaging and watches sales soar. Meanwhile, Bangda continues swimming with complete indifference to fashion. Mathi has never hired a marketing consultant. Anchovies remain blissfully unaware that they are nutritional celebrities. Some reputations are built through advertising. Others are built through evolution.

After all the research papers, capsules, advertisements and debates, the verdict remains refreshingly ordinary. A well-cooked fish on the dining table is not an outdated tradition waiting to be replaced by modern science. It is modern science repeatedly discovering why that tradition survived in the first place. Every generation invents new ways of describing good nutrition. The ocean has been quietly demonstrating it for millions of years.
